HEALTH VISITORS The health visitors in the practice are registered nurses with qualifications in midwifery, family planning and community health. They are here to provide information, advice and support on any health-related issue. They offer services to everyone registered in the practice. The health visitors with the practice are:
DISTRICT NURSES District nurses visit people in their own homes to give nursing care to those suffering acute, chronic or terminal illness and work with GPs to plan and provide appropriate, individualised care for both the patient and their family. This prevents unnecessary hospitalisation and enables early discharges to be picked up by the nursing team. Although the district nurses work largely in patients' homes, they also undertake treatment sessions for those who are unable to come to the surgery.
